Our agency, under the direction of the patient's physician, provides the care needed to adequately maintain patients at home. In the environment of their home, the patient will be cared for thoroughly by a staff of skilled professionals dedicated to maximizing the wellness state of the patient.
The objectives of the agency are to meet the physical and psychosocial needs of the patient during short-term or long-term illness, disability or emotional crisis, which may threaten their normal pattern of life. Services provided will enhance the individual's and family's lives.

Fostering Independence
Each patient admitted to the agency is evaluated by a professional nurse/case manager. The plan of care established by the case manager is evaluated by an interdisciplinary team on a regular basis, under the physician's direction. The plan of care is updated to meet the patient's changing needs. Our staff is thoroughly screened, periodically evaluated, and participates in organized in-service education programs to ensure the highest quality of care.
